I came across the remains of an RTA yesterday afternoon on my way home from work, it was all over bar the shouting. At the junction of the A57 and the A156 some idiot got himself into the wrong carriageway and hit another car head on.

I can't imagine any other than he was travelling towards Lincoln and turned right off the A156 onto the A57 and went the wrong side of the bollard and maybe something was coming from Lincoln and had already taken the slip road which avoids the traffic lights and this distracted him/her from what was right in front.
It's a strange junction and you really need your wits about you there. Some images from street view show the layout.

It is Mike, I nearly got cleaned up there last year. I was on my way home from work (in Shirl's car:wideyed:) after taking it for test, when a woman was waiting in the "box" to turn right down the A57 I she sort expected the lights had turned to red and I was stopping, but I wasn't but, by God, I bloody had to. The look on her passenger's face was a picture too, watching me slithering towards him shouting obscenities.
Should have been a roundabout, then at least they wouldn't have had the local Pikeys (Rooneys) connecting their grow house into the traffic lights=))

The U.K. feautures amazingly bad layouts despite spending more than the entire North and South of America on paint, bollards, traffic lights and other sh*** they call 'street' furniture.

You ought to see it over here, its like toy town with all the markings on the roads.
 
They resurfaced the main road past our place in Kissimmee last year and I think they must've done the road markings in white emulsion paint because when it rains you just can't bloody see them and what is left is almost scrubbed off. Must need a few more tourists to help pay for it I guess.;)
