Anyone know whats going on here in these pics?...looks like some kind of holes behind the speaker grill?...do they just pop off or does the whole door card need to come off?20200312_132537.jpg 20200312_132542.jpg 20200312_132537.jpg 20200312_132542.jpg 20200312_132546.jpg

Yeah, the original speakers [probably deteriorated/rotted away and where removed. Mine are like that, the front speaker (larger hole) falls apart first, the rear speaker I replaced with a tweeter. I should replace the larger one too but haven't found a speaker that small that has good sound since it's not a tweeter and is a mid or high-mid frequency speaker.
To continue, BMW uses a 4-way design with the HK system, there are 2 speakers in the door, one in the kick panel and the sub. There are also 2 full-range or almost full range speakers behind the seats. Conventional car audio design tends to be a 3-way design for the high-end stuff or it use to be.
So what I may do for my vehicle is delete the OEM amplifier and convert to a 3-way system with matching amplifiers, It's just I've been too lazy to have done this yet, someday I will.

The reason for the extra hole is the Harman Kardon option, it’s an amplified system and there is a mid and tweeter in the door as well as the mid Bass in the foot wells and two rear fill speakers behind the seats and a sub in the rear console.

I think all you are seeing here is the speaker felt rotting away. It happens on all cars due to ageing.

Here is the assembly dismantled with the offending felt. Note there is an additional mesh shown from the other side.

I refurbished mine some months back, stripped them down like mrscalex has done, painted them black satin and replaced the felt from hobby-craft. The mesh grille is a little delicate in the removal and replacement, but totally transforms them once done. Unfortunately the door cards have to be removed.
Good luck.
